Job summary

Outer Cape Health Services, a community-focused primary care provider on Cape Cod, seeks a Primary Care Clinician to lead an interdisciplinary team and deliver comprehensive, continuous care to a patient panel.

You will collaborate with RNs, MAs, Behavioral Health, and care coordinators; participate in daily huddles, quality improvement, and value-based care initiatives; maintain documentation and participate in on-call coverage.

Qualifications

• M.D. or D.O., board certification or eligibility; Massachusetts license
• Graduation from an accredited program and current certification for NP/PA applicants
• BLS certification required; Valid DEA registration and prescribing authority as applicable

Responsibilities

• Lead and collaborate within an assigned care team (RNs, LPNs, Medical Assistants, Behavioral Health Clinicians, and Patient Access staff) to manage the health of a patient panel and ensure outcomes.
• Maintain an assigned patient panel and meet access and productivity expectations while balancing quality and continuity.
• Participate in or lead daily team huddles to anticipate patient needs, identify care gaps, and coordinate workflow.
• Provide clinical oversight, mentorship, and support to nursing and allied health staff to ensure team members work at the top of their licensure.
• Collaborate with Integrated Behavioral Health providers and Community Health Workers to address social determinants of health and mental health needs.
• Support same-day access, urgent care needs, and flexible scheduling to meet patient demand.
• Participate in patient care audits, performance improvement initiatives, population health activities, and value-based care efforts to improve outcomes.
• Complete documentation in a timely manner consistent with organizational standards and regulatory requirements.
