Job summary

Outer Cape Health Services seeks a Primary Care Clinician to lead and collaborate within an interdisciplinary care team at Orleans Health Center, MA. You will deliver comprehensive, continuous care to a patient panel, coordinate care, and align with our patient-centered medical home model.

Reporting to the Site Medical Director, you will mentor staff, ensure top-of-licensure practice, and participate in quality improvement, on-call duties, and weekend coverage as needed.

Qualifications

  • Practice within the scope of licensure and certification consistent with Massachusetts law and OCHS policy.
  • For physician applicants: M.D. or D.O., board certification or eligibility.
  • For NP/PA: Graduation from an accredited program and current certification.
  • BLS certification required.
  • Valid DEA registration and prescribing authority as applicable.
  • Eligible to participate in Medicare, Medicaid, and other third-party payer programs.

Responsibilities

  • Lead and collaborate within an assigned care team to manage the health of a patient panel and champion shared accountability for outcomes.
  • Maintain an assigned patient panel and meet access and productivity expectations while balancing quality, continuity, and team-based care principles.
  • Participate in and/or lead daily team huddles to anticipate patient needs, identify care gaps, and coordinate workflow.
  • Provide clinical oversight, mentorship, and support to nursing and allied health staff to ensure team members work at the top of their licensure.
  • Collaborate with Integrated Behavioral Health providers and Community Health Workers to address social determinants of health and mental health needs.
  • Support same-day access, urgent care needs, and flexible scheduling strategies to meet patient demand.
  • Participate in patient care audits, performance improvement initiatives, population health activities, and value-based care efforts to improve clinical quality and outcomes.
  • Complete documentation in a timely manner consistent with organizational standards and regulatory requirements.
  • Collaborate with leadership and staff on care delivery improvements, workflows, and quality initiatives.
  • Participate in provider meetings, team meetings, and organizational initiatives as directed by the Site Medical Director or Chief Medical Officer.

Primary Purpose of Position:

Reporting to the Site Medical Director, the Primary Care Clinician serves as a clinical leader within an interdisciplinary care team and provides comprehensive, continuous care to a panel of patients. The clinician collaborates with team members to coordinate care, promote health, manage chronic disease, and improve patient outcomes in alignment with OCHS's mission and team-based care model.

Essential Functions:
  • Lead and collaborate within an assigned care team (including RNs, LPNs, Medical Assistants, Behavioral Health Clinicians, and Patient Access staff) to manage the health of a patient panel and champion shared accountability for outcomes.
  • Maintain an assigned patient panel and meet access and productivity expectations while balancing quality, continuity, and team-based care principles.
  • Participate in and/or lead daily team huddles to anticipate patient needs, identify care gaps, and coordinate workflow.
  • Provide clinical oversight, mentorship, and support to nursing and allied health staff to ensure team members work at the top of their licensure.

Qualifications
  • Practice within the scope of licensure and certification consistent with Massachusetts law and OCHS policy.
  • Advanced Practice Clinicians (NPs/PAs) must maintain appropriate supervision/collaborative agreements as required.
  • For physician applicants: M.D. or D.O., board certification or eligibility in an appropriate specialty.
  • For NP/PA applicants: Graduation from an accredited program and current certification.
  • Possess and maintain a valid and unrestricted license to practice in the Commonwealth of Massachusetts.
  • BLS certification required.
  • Valid DEA registration and prescribing authority as applicable.
  • Eligible to participate in Medicare, Medicaid, and other third-party payer programs.
  • Demonstrated commitment to team-based care and the Patient-Centered Medical Home model.
  • Strong leadership, collaboration, and communication skills.
