The role emphasizes population health leadership, accreditation maintenance, and cross‑functional teamwork within a Patient‑Centered Medical Home model in a community‑based setting.

Qualifications

• Practice within MA licensure rules and OCHS policy.
• For physicians: MD/DO with board certification or eligibility in an appropriate specialty.
• For NP/PA: graduated from accredited program with current certification.
• Must possess and maintain a valid MA license to practice.
• BLS certification required.
• DEA registration and prescribing authority as applicable.
• Eligible to participate in Medicare, Medicaid, and other third‑party payer programs.
• Demonstrated commitment to Patient‑Centered Medical Home model.
• Strong leadership, collaboration, and communication skills.
• Proficiency in EPIC, Outlook, Microsoft Office, and OneDrive.

Responsibilities

• Provide comprehensive, continuing care including diagnosis and treatment with referrals as needed.
• Lead a clinic team and manage health of patient panel.
• Supervise clinicians and allied staff to ensure high-quality care.
• Participate in patient care audits for quality assurance.
• Oversee complex clinical cases and provide direct consultations.
• Contribute to staff performance reviews and hiring decisions.
• Participate in on-call and weekend coverage as needed.
• Maintain Board Certification and CME for licensure.
• Attend regular staff meetings and participate in policy development.
