Job summary

Socket.dev is seeking a Care Coordinator to manage patient care journeys within a clinic-based setting in the Natomas/Sacramento area. The role collaborates with patients, healthcare providers, and external payers to ensure access to necessary services and continuous support throughout the healthcare journey.

The candidate should demonstrate strong organization, excellent communication, and a patient-centered approach, with experience working with HMO or IPA payers or care coordination functions.

Qualifications

• Strong organizational and time-management abilities.
• Excellent written and verbal communication skills.
• Proficient in Google Suite (Sheets and Docs).
• Strong problem-solving abilities and attention to detail.
• Compassionate and patient-centered approach to care.
• 1+ years experience working with HMO or IPA payers or care coordination responsibilities.

Responsibilities

• Referrals and authorizations coordination for specialist care, diagnostics, imaging, etc.
• Care coordination with IPA or HMO payers, members and patients for preventative care and access to services.
• IPAs coordinating care for establishing care in primary care (PC).
• Provider coordination and patient engagement; manage provider messages and documentation.
