Elk Ridge Community Health is seeking a Behavioral Health Consultant to join our primary care team. The role provides assessment and intervention for psychosocial concerns linked to medical conditions, coordinating with a multidisciplinary group to promote cohesive, whole-health care.

Expect collaboration with primary care providers to support integrated treatment plans. The BHC will maintain confidential records, deliver education to teams, and participate in ongoing training and emergency

Competenze

• Master’s degree required in Social Work, Counseling, Psychology or related field.
• Three (3) years post-graduate experience with behavioral health patients in a clinical setting.
• Current Arizona Behavioral Health License at the independent level (LPC, LCSW, Clinical Psychologist).
• Current Basic Life Support (BLS) certification for health care providers.
• Fingerprint Clearance Card and Level I clearance or eligibility within seven working days.

• Provide focused quality clinical services addressing patients’ health and behavioral concerns.
• Coordinate care and referrals with medical providers and multidisciplinary team.
• Maintain accurate electronic health records in compliance with HIPAA and organizational policies.
• Educate primary care and behavioral health teams on latest behavioral health assessment and intervention knowledge.
• Assist patients with crisis navigation and staff mobile crisis teams when needed.
• Participate in Behavioral Emergency Response Team (BERT) for rapid intervention and safety.
