Millennium Physician Group in Florida is seeking an Advanced Practice Provider (APRN or PA) to deliver total patient care in collaboration with the physician and care team. The APP will assess patients, create care plans, prescribe, and manage care across acute, chronic, and palliative needs with timely documentation.

License in good standing in Florida is required. The role includes coordination with care providers, guiding patient-centered care, and supporting quality measures and end-of-life

Qualifications

• Total care for assigned patients in collaboration with the physician and patient care team.
• Conduct patient assessments and develop care plans with risk management and quality focus.
• Document timely in the electronic medical record per CMS and MPG policies.
• Diagnose, treat, and order tests, prescriptions, and therapies within scope.
• Coordinate with home health, social services, and community resources as needed.

Responsabilités

• Provide comprehensive patient care across acute, chronic, and palliative needs.
• Perform patient assessments, create care plans, and implement interventions.
• Communicate with care team and supervise related medical assistants as applicable.
• Ensure CMS quality measures and HCCs are addressed per guidelines.
• Maintain ongoing collaboration with the supervising physician for complex decisions.
