Job summary

Trinity Health IHA Medical Group in Waterford, MI is seeking a passionate APP (NP/PA) to provide comprehensive outpatient primary care for patients of all ages. You will perform preventive care visits, manage acute illnesses, and follow chronic conditions within a collaborative, multidisciplinary team using EPIC for documentation and care coordination.

Qualifications

Board Certified/Eligible Family Medicine NP/PA (or equivalent) with Michigan license and DEA.
Experience delivering primary care to patients of all ages is preferred.
Comfort with EPIC or similar EMR systems and collaborative care teams.

Responsibilities

Provide comprehensive outpatient primary care to patients of all ages.
Conduct preventive, acute, and chronic care visits.
Collaborate with physicians, behavioral health, care managers, and nursing staff.
Utilize EPIC for documentation and care coordination.
Participate in quality improvement and value-based care initiatives.
