Trinity Health IHA Medical Group in Waterford, MI is seeking a passionate APP (NP/PA) to provide comprehensive outpatient primary care for patients of all ages. You will perform preventive care visits, manage acute illnesses, and follow chronic conditions within a collaborative, multidisciplinary team using EPIC for documentation and care coordination.

Qualifications
- Board Certified/Eligible Family Medicine NP/PA (or equivalent) with Michigan license and DEA.
- Experience delivering primary care to patients of all ages is preferred.
- Comfort with EPIC or similar EMR systems and collaborative care teams.
Responsabilités
- Provide comprehensive outpatient primary care to patients of all ages.
- Conduct preventive, acute, and chronic care visits.
- Collaborate with physicians, behavioral health, care managers, and nursing staff.
- Utilize EPIC for documentation and care coordination.
- Participate in quality improvement and value-based care initiatives.
