Job summary

Southeast Primary Care Partners is seeking an Advanced Practice Provider (PA/NP) to join our Primary Medical Group in a collaborative, patient-centered care model. You will work with Family Practice/Internal Medicine physicians to manage patient populations in outpatient settings, with potential inpatient duties as needed.

The role emphasizes evidence-based practice and performance metrics, with responsibilities across chronic and acute care, testing, treatment, and patient education.

Qualifications

• Graduate of an accredited PA or NP program.
• Current licensure and prescriptive authority through the Georgia Board (or equivalent state board).
• Current BLS

Responsibilities

• Diagnose and manage acute and chronic conditions.
• Order and interpret diagnostic tests (labs, X-ray, EKG).
• Prescribe medications and treatments.
• Educate patients about conditions, plans, and outcomes.
• Collaborate within a multidisciplinary team to deliver evidence-based care.
• Perform additional duties to support patient care and organizational objectives.
• Participate in limited on-call duties, primarily phone-based.
