Job summary

Memorial Health in Springfield, IL is seeking a Nurse Practitioner or Physician Assistant to join the team at Koke Mill Family Medicine. This Family Medicine Advanced Practice Provider position treats patients from pediatric to geriatric and focuses on delivering evidence-based, comprehensive care under physician supervision.

You will collaborate with other providers, refer as needed, and support patient advocacy, teaching, and family counseling.

Qualifications

• Licensure as an APRN or PA per IL state law.
• Maintain IL Controlled Substance license within 3 months of hire.
• Maintain DEA license within 3 months of hire where required.
• CPR certification current; ACLS where required.

Responsibilities

• Treat patients across the lifespan in Family Medicine.
• Coordinate and evaluate evidence-based comprehensive care.
• Collaborate and refer to other health care providers; educate families.
• Provide teaching and family counseling under physician direction.

Education

APRn license (IL) or PA license (IL)

APRN or PA certification as determined by department

Job Description Summary

Great opportunity for a Nurse Practitioner or Physician Assistant to join our Team at Koke Mill Family Medicine, located in Sprinfield, IL! This is a Family Medicine Advanced Practice Provider position, responsible for treating patients of all ages across the lifespan (pediatric through geriatric). This role will facilitate the delivery and evaluation of evidence-based comprehensive care to patients. Collaboration and referral to other health care providers, patient advocacy, teaching, and family counseling are major components of the role. Under the direction physician or designee, provides coordination of their respective department or work area.

Licensure/Certification/Registry

• One of the following is required:
• Must possess and maintain licensing as an APRN as required by Illinois State Law;
• Must possess and maintain licensing as a PA as required by Illinois State law;
• Appropriate certification as a APRN or PA, as determined by department.
• Maintain IL Controlled Substance license (Must be obtained within 3 months of hire if not held at time of hire).
• Maintain DEA license (Must be obtained within 3 months of hire if not held at time of hire) in positions where required.
• Valid CPR certification required.
• ACLS where required.

Experience

Minimum of 1-year post-graduate as a mid-level provider preferred
