Job summary

Memorial Health in Decatur, IL, seeks a Family Medicine Advanced Practice Provider (APN/PA) to join West Hay Family Medicine on-site. You will care for patients from pediatrics to geriatrics, deliver evidence-based care, and collaborate with physicians. A $10,000 sign-on bonus is offered.

Under physician direction, you’ll coordinate services, teach, and provide patient advocacy and family counseling as part of a comprehensive care team. Illinois licenses and certifications are required.

Qualifications

• Must possess and maintain APRN or PA licensure per Illinois law.
• Maintain IL Controlled Substance license within 3 months of hire.
• Maintain a DEA license within 3 months where required.
• CPR certification; ACLS where required.
• Experience as a mid-level provider preferred.

Job Description Summary: $10,000 Sign On Bonus Opportunity Great opportunity for a Nurse Practitioner or Physician Assistant to join our Team at West Hay Family Medicine, located in Decatur, IL! This is a Family Medicine Advanced Practice Provider position, responsible for treating patients of all ages across the lifespan (pediatric through geriatric). This role will facilitate the delivery and evaluation of evidence-based comprehensive care to patients. Collaboration and referral to other health care providers, patient advocacy, teaching, and family counseling are major components of the role.

Under the direction physician or designee, provides coordination of their respective department or work area.

Licensure/Certification/Registry:

• One of the following is required:
• Must possess and maintain licensing as an APRN as required by Illinois State Law;
• Must possess and maintain licensing as a PA as required by Illinois State law;
• Appropriate certification as a APRN or PA, as determined by department.
• Maintain IL Controlled Substance license (Must be obtained within 3 months of hire if not held at time of hire).
• Maintain DEA license (Must be obtained within 3 months of hire if not held at time of hire) in positions where required.
• Valid CPR certification required.
• ACLS where required.

Experience:

• Minimum of 1-year post-graduate as a mid-level provider preferred
• Pay Transparency $0.01 - $0.01 Additional compensation in the form(s) of participation in our annual incentive program are dependent on role. Pay is based on factors such as work experience, education, certification(s), training, skills, and competencies related to the role.
