Memorial Health in Springfield, IL is seeking a Nurse Practitioner or Physician Assistant to join Koke Mill Family Medicine. This Family Medicine Advanced Practice Provider role serves patients from pediatric to geriatric, delivering evidence-based, comprehensive care.

Under physician direction, you will coordinate care, collaborate with providers, teach, and counsel families. Active APRN/PA licensure per Illinois law, CPR, and required certifications are essential.

Qualifications

• Must possess and maintain APRN or PA licensure as required by Illinois State Law.
• Maintain IL DEA license (if required) and Controlled Substance license within 3 months (if not held at time of hire).
• Maintain CPR certification.
• ACLS where required.

Responsibilities

• Treat patients of all ages across the lifespan in family medicine.

Job Description Summary

Great opportunity for a Nurse Practitioner or Physician Assistant to join our Team at Koke Mill Family Medicine, located in Sprinfield, IL! This is a Family Medicine Advanced Practice Provider position, responsible for treating patients of all ages across the lifespan (pediatric through geriatric). This role will facilitate the delivery and evaluation of evidence-based comprehensive care to patients. Collaboration and referral to other health care providers, patient advocacy, teaching, and family counseling are major components of the role. Under the direction physician or designee, provides coordination of their respective department or work area.

Licensure/Certification/Registry

• One of the following is required:
• Must possess and maintain licensing as an APRN as required by Illinois State Law;
• Must possess and maintain licensing as a PA as required by Illinois State law;
• Appropriate certification as a APRN or PA, as determined by department.
• Maintain IL Controlled Substance license (Must be obtained within 3 months of hire if not held at time of hire).
• Maintain DEA license (Must be obtained within 3 months of hire if not held at time of hire) in positions where required.
• Valid CPR certification required.
• ACLS where required.

Experience

Minimum of 1-year post-graduate as a mid-level provider preferred
