Memorial Health is seeking a Part-Time Advanced Practice Provider for our South Sixth Street Primary Care Team in Springfield, IL. The role runs 8:00am–5:00pm, Monday to Friday, with no weekends, holidays, or on-call requirements.

You will treat patients of all ages under physician supervision and coordinate comprehensive care across the lifespan. The APRN or PA must hold Illinois licensure with CPR (and ACLS where required).

Great opportunity for a Nurse Practitioner or Physician Assistant to join our South Sixth Street Primary Care Team, located in Springfield, IL! Part-Time Day Shift (8:00am - 5:00pm), No Weekends/Holidays/Call This is a Primary Care Advanced Practice Provider position, responsible for treating patients of all ages across the lifespan (pediatric through geriatric).

This role will facilitate the delivery and evaluation of evidence-based comprehensive care to patients. Collaboration and referral to other health care providers, patient advocacy, teaching, and family counseling are major components of the role.

Under the direction physician or designee, provides coordination of their respective department or work area.

Licensure/Certification/Registry

• Must possess and maintain licensing as an APRN as required by Illinois State Law;
• Must possess and maintain licensing as a PA as required by Illinois State law
• Appropriate certification as a APRN or PA, as determined by department.
• Maintain IL Controlled Substance license (Must be obtained within 3 months of hire if not held at time of hire).
• Maintain DEA license (Must be obtained within 3 months of hire if not held at time of hire) in positions where required.
• Valid CPR certification required.
• ACLS where required.

Experience

Minimum of 1-year post-graduate as a mid-level provider preferred.

Pay Transparency

$0.01 - $0.01 Additional compensation in the form(s) of participation in our annual incentive program are dependent on role. Pay is based on factors such as work experience, education, certification(s), training, skills, and competencies related to the role.
