Job summary

Qualifications

• Master’s degree or higher in nursing or physician assistant studies.
• Current Illinois license as a nurse practitioner or physician assistant.
• Certification by ANCC or NCCPA.
• Minimum of two years of experience as an APP in primary care preferred.
• Current BLS and ACLS certifications.
• Proficiency in EMR systems.

Responsibilities

• Perform patient assessments and assign diagnosis based on data and current research.
• Establish care plans and implement interventions per care paths and standards of care.
• Provide education and counseling to patients, families and groups.
