Qualifications

• Licensed APRN/PA/CNS with current credentialing in applicable state(s).
• National certification for APRN/PA/CNS as appropriate.
• BLS and advanced life support certifications as required by role.
• Ability to work in a multidisciplinary team and coordinate care.

Responsibilities

• Take thorough medical histories and perform comprehensive physical exams across the lifespan.
• Order and interpret labs and imaging; develop evidence-based treatment plans.
• Prescribe medications per state licensure and document care in EMR.
• Educate patients on disease management and preventive care; participate in rounds.
• Refer to specialists and coordinate care across the health system.
