• Must be graduate of an accredited APRN/PA/CNS program.
• Ambulatory care experience preferred.
• Able to work across multiple ambulatory care departments as needed.
• Eligible for state licensure and board certification as applicable.

Responsibilities

• Conducts comprehensive medical histories and exams, orders tests, and develops a working diagnosis and treatment plan.
• Performs routine and specialized procedures as credentialed.
• Prescribes and monitors medications as allowed by licensure.
• Educates patients and coordinates care with the healthcare team.
• Facilitates continuity of care and discharge planning when needed.
