Job summary

Portage Path Behavioral Health, Inc. in Akron, OH, is seeking a highly skilled Advanced Practice Provider (NP or PA) for our Primary Care Team.

This contract role provides 8 to 16 hours weekly, Monday through Friday, with no holidays or weekends, delivering comprehensive primary care within a community behavioral health setting. You will perform exams, manage acute and chronic conditions, order tests, prescribe within regulations, and collaborate with a multidisciplinary team.

Qualifications

• Graduate of an accredited NP or PA program.
• Current state licensure and national certification.
• Current DEA license or eligibility.
• Current BLS certification.
• Experience in family medicine, community health, or primary care (preferred).
• Strong commitment to serving medically underserved populations.

Responsibilities

• Provide comprehensive primary care services including assessment, diagnosis, and treatment of acute and chronic conditions.
• Conduct physical exams and obtain medical histories.
• Order and interpret laboratory and diagnostic tests.
• Prescribe medications and treatments in accordance with state and federal regulations.
• Provide preventive care including wellness exams, screenings, and immunizations.
• Manage chronic diseases such as diabetes, hypertension, and asthma.
• Educate patients on disease prevention, lifestyle modifications, and medication adherence.
• Collaborate with physicians, behavioral health providers, dentists, and care coordinators in a team-based care model.
• Document care accurately in the electronic health record (EHR).
• Participate in quality improvement initiatives and population health efforts.

Job Overview

We are seeking a highly skilled and compassionateAdvanced Practice Provider (Nurse Practitioner or Physician Assistant)for our Primary Care Team. This role involves delivering high-quality patient assessments, managing acute and chronic conditions, and supporting our mission to provide effective, affordable mental health and medical services within a community setting.This position is contract, seeking a minimum of 8 to 16 hours of weekly coverage (Monday – Friday, no holidays and no weekends).
