Job summary

Central Ohio Primary Care Physicians, Inc. seeks a full-time licensed NP/PA to join Grandville Pike Family Physicians in Lancaster, OH. You will deliver comprehensive primary care in an outpatient setting to patients of all ages.

The role focuses on preventive medicine, chronic disease management, well visits, vaccinations, and patient education. Collaboration with physicians and a multidisciplinary team is essential; EPIC experience is a plus.

Qualifications

• Master’s degree in nursing or physician studies is required.
• Current licensure as a Nurse Practitioner in Ohio is required.
• National certification as a Family Nurse Practitioner (FNP) is required.
• Minimum 1 year of outpatient practice experience preferred.
• EPIC EHR experience is helpful; training provided at onboarding.

Responsibilities

• Conduct comprehensive assessments including medical history reviews and physical exams to diagnose and treat acute and chronic conditions.
• Provide full primary care services: well visits, health promotion, disease prevention, vaccines, and management of common illnesses.
• Develop individualized treatment plans for chronic diseases (diabetes, hypertension, asthma, COPD) and monitor progression.
• Educate patients and families on health maintenance, prevention, and treatment options; promote informed decision-making.
• Coordinate patient care with physicians and multidisciplinary team; facilitate referrals as needed.
• Maintain accurate documentation of encounters and treatment plans using EPIC in compliance with standards.

COPC is the largest independent physician-owned primary care group in the nation. They are seeking an experienced full-time licensed board-certified NP/PA to join their team at Grandville Pike Family Physicians, 1800 Granville Pike, Lancaster, Ohio 43130.

This practice is about 30 miles from downtown Columbus and about a 20-30-minute drive from neighboring towns.

About the practice:

This well-established practice has 4 physicians, an Office Manager, an APP, medical assistants and service reps to support the providers and patients.

This Primary Care Group is an outpatient practice with a great reputation for patient care in the community. Its focus is on health education, preventive medicine, and disease management to deliver high-quality patient care that is the Best for Primary Care.

Office Hours: 8:00 - 5:00 pm Mon-Friday.

Key Responsibilities:

Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages.

Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries.

Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, medication management and educate patients on self-management strategies.

Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.

Collaborate with physicians, advanced practice providers, nurses to coordinate patient care across multiple disciplines and healthcare settings. Facilitate referrals to specialists and other healthcare providers as needed.

Maintain accurate and timely documentation of patient encounters, treatment plans, and follow-up care using electronic health record systems in compliance with regulatory standards and organizational policies. COPC uses EPIC.

Qualifications:

Master’s degree in nursing or physician studies from an accredited program.

Current licensure as a Nurse Practitioner in Ohio.

National certification as a Family Nurse Practitioner (FNP).

Preferred: A minimum of 1 year of practical working experience as an NP post-graduation in an outpatient practice setting.

Strong clinical assessment, diagnostic, and treatment skills.

Excellent communication and interpersonal skills.

Ability to work independently and collaboratively within a multidisciplinary team.

EPIC EHR experience is helpful. Training is available at onboarding.
