Odyssey House Louisiana, Inc. is seeking a full-time Case Manager for the Prevention Program in Louisiana. The schedule is Monday-Friday 8am-5pm.

This role is a full-time grant funded position under the Hepatitis C Elimination Initiative Pilot, supporting participants with medical, behavioral health, and social service systems. The Case Manager provides peer-based support, advocacy, and individualized care coordination to empower long-term health and stability.

Qualifications

• Understanding of at-risk communities.
• Excellent interpersonal and communication skills.
• Experience in care coordination and federal, state benefit enrollment.
• Excellent communication and organizational skills.

Responsibilities

• Assess client needs, connect to services, support Medicaid enrollment, coordinate care, document progress.
• Assess Medicaid status and follow policy to obtain and maintain status.
• Make referrals to fill gaps in assistance.
• Screening, intake, assessment, housing plan development, monitoring, and linkage to resources.
• Deliver health education programs and community outreach through grant initiatives.
• Provide support and care coordination to navigate medical and social services.
