Community Healthcare Network, Inc. is seeking a Medical Care Specialist to support the expansion and delivery of PrEP/PEP services within the BHACS TelePrEP program.

You will conduct virtual and in-person intakes, coordinate referrals, assist clients with insurance navigation, and help maintain client records. Collaborating with medical providers and program staff, you will develop care plans, provide adherence counseling, and ensure services align with health literacy and cultural competency,

Qualifications

• Complete intake and initial and annual psychosocial assessment.
• Work closely with Program Manager to ensure TelePrEP Program services are offered according to program deliverables.
• Provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient need.
• Develop, implement and coordinate a medical care plan with the client’s health

Responsabilités

• Maintain accurate and detailed records and ensure providers are informed of client progress.
