Job summary

Northeast Valley Health Corporation is seeking a Health Educator I for the PCHP to implement grant deliverables and engage high‑risk individuals in PrEP. You will use a validated HIV risk screen, support outreach efforts, and collaborate with the PCHP team to improve health outcomes in the LA area.

The role requires bilingual fluency (English/Spanish), experience with LGBTQ+ HIV prevention, and strong communication skills for providers and community partners.

Qualifications

• Bachelor's degree in Public Health or related field is required.
• Two years of experience working with LGBTQ+ populations in HIV prevention.
• Fluency in English (speaks, reads, writes) and Spanish (speaks, reads, writes).
• Strong verbal and written communication with medical and behavioral health teams.
• Knowledge of HIV/STI disparities, harm reduction, and related outreach.
• Valid California driver's license with acceptable insurance.
• Ability to conduct evening and weekend community outreach on short notice.

Responsibilities

• Implement grant deliverables for the PCHP project under supervision.
• Use validated HIV risk screening to identify patients needing PrEP.
• Develop and execute in-reach and outreach campaigns for high‑risk individuals.
• Provide training on PrEP benefits to providers and clinical staff.
• Assist with data collection for program metrics and reports.
• Participate in community events to recruit PrEP clients.
