Job summary

Chase Brexton Health Care in Maryland is seeking a Nurse Care Manager to provide patient-centered nursing care in ambulatory settings under supervision. The nurse will perform direct nursing and telephonic services, collect and analyze patient data, assist with care plans, and evaluate outcomes within a multidisciplinary team.

Qualifications

Must be a graduate of an accredited nursing program and hold a Maryland RN license.
CPR certification required by start date.
Experience with ambulatory or acute care setting preferred.
Strong documentation in EMR required.

Responsibilities

Assess patient needs and develop care plans as part of the multidisciplinary team.
Facilitate individual or group nursing visits for chronic illnesses.
Identify patients at risk and provide outreach and follow up.
Provide complex case management and medication adherence support.
Perform medication reconciliation and assess barriers to adherence.
Triage patients during walk-in or same-day clinic.
Collaborate with providers and transitions of care teams.
Ensure compliance with Joint Commission and PCMH standards.
