Job summary

Yochana is seeking a detail-oriented Medical Coder to review medical records and assign accurate diagnostic and procedure codes. You will ensure coding follows established guidelines to support billing, reimbursement, and healthcare reporting.

Responsibilities include reviewing records, assigning ICD-10-CM, CPT, and HCPCS codes, verifying information for accuracy, and maintaining confidentiality in a HIPAA-compliant environment. 1+ year in medical coding is preferred.

Qualifications

• High school diploma or equivalent; healthcare or medical coding education preferred.
• CPC/CCS/CCA or equivalent medical coding certification preferred.
• 1+ year of medical coding or healthcare revenue-cycle experience.

Responsibilities

• Review patient records, physician documentation, and notes.
• Assign ICD-10-CM, CPT, and HCPCS codes with accuracy.
• Verify diagnosis and procedure information for accuracy.
• Ensure coding complies with CMS, payer, and organizational guidelines.
• Identify missing or unclear documentation and seek clarification.
• Review claims and coding data for errors or inconsistencies.
• Maintain patient confidentiality and HIPAA compliance.
• Assist with coding audits and quality checks.
