Company Description

Divine Hands Home Health and Divine Hands Medical Servicesis seeking an experienced, organized, and compassionatePractice Managerto oversee operations for our home health agency and home-based primary care services. This position plays a vital role in ensuring smooth day-to-day operations, regulatory compliance, and high-quality patient care delivery.

Job details:

Part time/Full time: 24-40 hours/week

Pay rate:$35 - $38 per hour

Qualifications

  • Proven experience in healthcare operations or practice management (Home Health or Primary Care preferred).
  • Knowledge of CMS requirements for home health, CCM, RPM, and telehealth services
  • Strong knowledge of Medicare, Medicaid, and private insurance guidelines
  • Excellent leadership, communication, and organizational skills
  • Proficiency with EMR systems and healthcare billing platforms (Axxess and Tebra.Kareo)
  • Bachelor's degree in Healthcare Administration, Business, or related field (preferred)

Key Responsibilities

  • Manage daily operations of both home health and home-based medical practice
  • Supervise administrative staff, scheduling, billing, and intake coordination
  • Ensure compliance with state, federal, and payer regulations
  • Monitor and optimize workflow, documentation, and provider efficiency
  • Assist with credentialing, staff onboarding, and training
  • Support billing processes and insurance verification
  • Serve as liaison between clinical team, referral sources, and clients
  • Oversee credentialing, onboarding, and HR-related functions
  • Track key performance metrics and assist with strategic growth initiatives
