Job description

The Western New York VA Medical Center is seeking a Licensed Practical Nurse to work on the PACT Team at the Buffalo Location.

The Licensed Practical Nurse (LPN) works under the supervision of the registered nurse, providing a full range of practical nursing care to patients with various physical and behavioral problems.

Responsibilities

1. Establish positive relationships with patients, families, and staff using effective communication skills.
2. Assist in collecting and documenting patient data.
3. Participate in patient conferences and shift reports.
4. Maintain a safe, clean, and therapeutic environment following safety standards.
5. Provide competent patient care ensuring safety for all.
6. Assist with diagnostic measures and specimen collection.
7. Respond to emergencies, initiate CPR, and maintain BLS certification.
8. Administer medications and document appropriately.
9. Record patient information and educate patients and families.
10. Protect patient privacy and confidentiality.
11. Participate in quality improvement activities.
12. Follow safety practices and report unsafe conditions.
13. Maintain professional boundaries.
14. Perform other duties as assigned.
