Job summary

Grace Skilled Nursing & Therapy is seeking a licensed nurse (RN or LPN) to fill the PPS Skilled Case Manager role. You will coordinate assessments, guide care planning, and ensure quality outcomes for residents, bridging clinical care with payer requirements.

The role emphasizes teamwork and effective communication to meet payer needs while delivering compassionate, resident-centered service. This is a full-time position with competitive benefits.

Qualifications

• Current Oklahoma RN or LPN license is required.
• Associate's degree with appropriate certification or BSN preferred.
• Experience as a Charge Nurse in long-term care and with MDS is a plus.
• Minimum 2+ years in MDS/LTC or acute case management preferred.

Job description

Our long-term care facility is actively seeking a detail-oriented and dynamic licensed nurse (RN or LPN) to be our new PPS Skilled Case Manager!

In this role, you will coordinate assessments, gather information, guide care planning, and ensure quality outcomes for our residents. The PPS Skilled Case Manager plays a key part in bridging clinical care with payer requirements, all while supporting our mission of compassionate, resident-centered service.

Responsibilities and Purpose

• Oversee gathering of information for assessments and care plans.
• Provide maximum flexibility in coordinating the payer's needs with patient outcomes.
• Communicate effectively with the treatment team and influence the treatment approach, when needed.
• Work cooperatively as a member of a team.

Experience and Education

• Current and unencumbered Oklahoma Registered Nurse (RN) or Licensed Practical Nurse (LPN) License.
• Associate's degree with appropriate certification or BSN.
• Experience as a Charge Nurse within long-term care.
• Experience with MDS and care planning process preferred.
• 2+ years of experience in MDS & LTC or Acute Case Managementpreferred.
