Job summary

Jefferson Health in Philadelphia is seeking a Transplant Coordinator to manage care for kidney and/or pancreas transplant recipients from the time of transplant and beyond. The role collaborates with the transplant team and referring physicians to monitor progress and deliver post-transplant education.

The position requires RN licensure, a Bachelor's degree (preferred), and at least 3 years of experience with renal/transplant patients.

Qualifications

• Bachelor's degree required or preferred.
• RN license required.
• ABTC certification preferred.
• Minimum 3 years experience with renal/transplant patients.
• Strong communication and writing skills.
• Proficiency with Microsoft Office applications.

Responsibilities

• Coordinate ongoing care of kidney and/or pancreas transplant recipients following transplantation.
• Monitor the progress and ongoing treatment of post-transplant recipients.
• Provide post-operative teaching and ongoing education to post-transplant recipients.
• Assist in the operation of post-transplant clinic.
• Interact with co-workers, visitors, and other staff consistent with hospital values.
