Job summary

Jefferson Health in Center City, Philadelphia, seeks a Post Kidney Transplant Coordinator (FT-Day, 40hrs/week) to coordinate care for kidney and/or pancreas transplant recipients. You will work with the transplant team and referring physicians to monitor progress and provide post-operative teaching.

Required: Registered Nurse with 3+ years in renal/transplant care; strong communication skills; proficiency with Microsoft Office. Bachelor’s degree preferred.

Qualifications

• Registered Nurse required; RN license active.
• Bachelor’s degree preferred for the nursing role.
• Minimum of three years’ experience with renal and/or transplant patients.
• Strong written and verbal communication; proficient in Office tools.

Responsibilities

• Coordinate ongoing care for kidney and/or pancreas transplant recipients.
• Monitor progress and treatment with transplant team and referring physicians.
• Provide post-operative teaching and ongoing education to recipients.
• Assist in operation of post-transplant clinic.
