Safe Children Coalition Inc. in Sarasota, FL seeks a Family Services Case Manager to deliver wrap-around referrals and supportive services to adoptive families and permanent guardians, tracking progress and coordinating behavioral health and community services to stabilize youth in the least restrictive care.

Responsibilities include facilitating family team meetings every 90 days, linking families to providers, crisis planning, documenting in the statewide system, and working with a supervisor.

Qualificações

• Bachelor's degree in psychology, social work, or related field or equivalent experience.
• 3+ years' experience working with troubled youth and families and/or mental health services, including compliance responsibilities.

Responsabilidades

• Provide wrap-around support and referrals to adoptive families and permanent guardians.
• Coordinate with community providers and services to stabilize youth.
• Facilitate family team meetings at least every 90 days.
• Assist with crisis and safety planning as needed.
• Document work in statewide computer system.
• Coordinate step-down and higher level of care referrals as needed.
