Job summary

AdventHealth Orlando is seeking a transplant coordination professional to organize referrals, assess eligibility, and manage patient education for advanced therapies and transplantation.

Responsibilities include coordinating multidisciplinary evaluations, maintaining records, and supporting families through treatment and follow-up care in a high‑intensity clinical environment.

Qualifications

• Associate degree preferred.
• Bachelor's degree preferred.
• 1+ year experience in a complex practice environment or healthcare setting coordinating multiple physician offices.
• Experience in critical care, dialysis, hepatology, cardiology, pulmonology or transplant clinical setting preferred.

Responsibilities

• Organizes and schedules appointments, tests, and consultations with transplant teams, surgeons, and cardiologists.
• Assesses patient eligibility for advanced therapy or organ transplantation by reviewing records and conducting preliminary tests.
• Coordinates initial referral processing and patient evaluations for advanced therapy, potential organ recipients, living donors or VAD patients.
• Provides education to patients and families about treatment process, risks, benefits and post-treatment care.
• Maintains accurate records of patient interactions, assessments and care plans; acts as liaison with the care team.
• Monitors progress post-treatment or post-transplant, schedules follow-ups, manages complications and provides ongoing support.
• Ensures adherence to medications and educates on side effects.
• Collaborates with providers to implement comprehensive care plans and supports emotional well-being.

Job Description:

Organizes and schedules necessary appointments, tests, and consultations with various healthcare providers, including transplant teams, surgeons, and cardiologists.Assesses patient eligibility for advanced therapy or organ transplantation by reviewing medical records, conducting preliminary tests, and consulting with specialists. Coordinates initial referral processing and patient evaluations for advanced therapy, potential organ recipients, living donors, or VAD patients. Provides detailed education to patients and families about the treatment process, potential risks, benefits, and post-treatment care. Maintains accurate and up-to-date records of all patient interactions, assessments, and care plans. Acts as a liaison between the patient, their family, and the healthcare team, ensuring effective communication and addressing any concerns. Monitors patient progress post-treatment or post-transplantation, schedules follow-up appointments, manages complications, and provides ongoing support. Ensures patients adhere to prescribed medications and treatments, educating them about compliance and managing side effects. Collaborates with healthcare providers to develop and implement comprehensive care plans. Offers emotional and psychological support to patients and families, addressing concerns and providing mental health resources. Participates in quality improvement initiatives to enhance patient care and outcomes. Stays informed about advancements in transplant medicine and incorporates evidence-based practices into patient care.Other duties as assigned.

Work Experience:

• 1+ experience in a complex practice environment or healthcare setting such as catheter lab, endoscopy, infusion center, etc., coordinating multiple physician office, with direct care or working in an inpatient care area within the last three years [Required]
• Experience in critical care, dialysis, hepatology, cardiology, pulmonology or transplant clinical setting [Preferred]

Licenses and Certifications:

• Certified Medical Assistant (CMA) [Preferred]
• Licensed Practical Nurse (LPN) [Preferred]
