Job summary

Regency Hospital, Northwest Indiana (East Chicago), is seeking a Clinical Liaison (RN/RRT) to advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals, coordinating care plans with physicians and families, and facilitating patient transfers.

You will help remove payer-related barriers for access to post-ICU care, working full-time with an extensive onboarding and training

Qualifications

• RN or RT license preferred; other designations may be considered on review.
• Ability to complete clinical assessments of medically complex patients.
• Experience in patient advocacy and coordination of post-ICU care is a plus.

Responsibilities

• Advocate for referred patients in pre-admission by collaborating with clinical teams.
• Coordinate care plans with physicians and families and facilitate transfers.
• Work to remove payer-related and operational barriers for admission to post-ICU care.
