Job summary

Select Specialty Hospital - Des Moines is seeking a Clinical Liaison (RN/RT) to advocate for referred patients in pre-admission, coordinate care plans with physicians and families, and facilitate transfers from ICU or post-ICU stays.

You will build relationships with referring facilities, work Monday–Friday 8–5, and earn a $10,000 sign-on bonus plus uncapped monthly bonuses, with extensive onboarding and generous benefits.

Qualifications

• RN or RT license required.
• Will consider other designations like LPN, PT, OT, upon review.
• Ability to complete clinical assessments of medically complex patients.

Responsibilities

• Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background, payer knowledge, and problem-solving skills.
• Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
• Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process through removing payer-related and operational barriers for patients getting into our highly effective level of care.
