Job summary

Lakeport Post Acute is seeking a compassionate Physical Therapist to join our skilled nursing and post-acute rehabilitation team in Lakeport, CA. This full-time position offers pay of $70-$75/hour, with travel/PRN options up to $95/hour, plus a $5,000 sign-on bonus and relocation assistance.

You will work under strong mentorship and leadership, with opportunities for growth and ongoing professional development.

Qualifications

• Licensed to practice physical therapy in California.
• Graduate of an accredited Physical Therapy program.
• SNF, Post-Acute, Rehabilitation, or Acute Care experience preferred.
• Strong communication and documentation skills.
• New graduates encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Deliver evidence-based therapy interventions.
• Monitor patient progress and adjust treatment plans as needed.
• Supervise PTAs and collaborate with the interdisciplinary team.
• Complete accurate and timely documentation; Educate patients and families on goals, exercises, and safety.
