Job summary

CareSource is seeking a Transitions Coordinator- Post Acute to monitor and manage member care transitions to long-term care, respite facilities, and HCBS waiver services through clinical review and benefits management. You will collaborate with ICT members, waiver coordinators, and providers to ensure appropriate supports during transitions.

The role requires RN licensure in Ohio, relevant case management experience, and the ability to coordinate documentation and enrollment across settings.

Qualifications

• Completion of an accredited Registered Nurse (RN) degree program or degree required to obtain Social Worker licensure in the State of Ohio.
• Bachelor of Science (B.S) in Nursing (BSN) or equivalent healthcare degree preferred.
• Three (3) years of experience in case management and/or managed care required.

Responsibilities

• Acts as point of contact with Care Coordinator, Waiver Service Coordinator, Medical Director, Provider, or other stakeholders within ICT to support coordination of level of care transitions.
• Ensures timely nursing facility-based level of care completion and HCBS waiver enrollment/disenrollment.
• Verifies eligibility and demographics during case reviews.
• Maintains benefit eligibility during level of care transitions.
• Prepares clinical documentation and supports care plans per established processes.
