Job summary

Buckeye Commons Care in Kent, Ohio is seeking a Healthcare Community Liaison to build relationships with hospitals, physician offices, and other referral partners. You will educate partners on our post-acute care services and connect patients and families with the right level of care.

The role focuses on developing a territory plan, growing census and revenue, and expanding partnerships across the community.

Qualifications

• Background in healthcare sales, business development, or related field
• Experience building/maintaining relationships with referral partners (hospitals, physician offices)
• Proven track record in territory development and census growth
• Excellent communication, organizational, and problem-solving skills
• Ability to work independently and as part of a team
• Strong networking and community outreach skills

Responsibilities

• Build and maintain relationships with hospitals, physician offices, and other referral partners to generate referrals and grow market share
• Develop and execute a territory development plan to increase census and revenue
• Identify opportunities to expand services and partnerships throughout the community
• Cultivate relationships with referral sources, including case managers and discharge planners
• Develop and maintain understanding of post-acute care options and services
• Provide education and training to referral partners on services and benefits
• Identify and capitalize on opportunities to improve patient outcomes and reduce healthcare costs
