Job summary

HCF Management, Inc. in Lima, Ohio seeks a Clinical Liaison to support the transition of care from acute to post-acute settings.

You will manage hospital referrals, communicate with families and clinicians, and help streamline patient transitions while traveling between hospitals as needed. The role emphasizes building relationships with hospital staff, maintaining up-to-date knowledge of our clinical services, and meeting census and referral targets.

Qualifications

• Licensed or Registered Nurse preferred, in good standing.
• Experience in care coordination and referral management.
• Excellent interpersonal communication skills, organizational skills and professional judgment.

Responsibilities

• Must follow HCF Policy and Procedures.
• Ensure continuity of care between providers for patients transitioning from acute to post-acute.
• Conduct clinical assessments and assist with collecting hospital documentation.
• Travel daily between hospitals; occasional travel to area SNFs.
• Evening and weekend availability.
• Establish, grow and maintain relationships with hospital-based referral sources.
• Maintain knowledge of our clinical services and local market conditions.
