Healthcare Community Liaison

Job Summary

Buckeye Commons Care is seeking a highly motivated and results-driven Healthcare Community Liaison to join our team in Kent, Ohio. As a Healthcare Community Liaison, you will be responsible for building relationships with referral partners, educating them on our services, and helping patients and families connect with the right level of post-acute care.

Responsibilities

• Build and maintain strong relationships with hospitals, physician offices, and other referral partners to generate referrals and grow market share

• Develop and execute a territory development plan to increase census and revenue

• Identify opportunities to expand our services and partnerships throughout the community

• Cultivate relationships with referral sources, including case managers, discharge planners, and other healthcare professionals

• Develop and maintain a comprehensive understanding of Sweetwater Care's services and post-acute care options

• Provide education and training to referral partners on Sweetwater Care's services and benefits

• Identify and capitalize on opportunities to improve patient outcomes and reduce healthcare costs
