Job summary

Rejuvenation Medical, PLLC is seeking an ARNP or PA-C for post-acute and long-term care across skilled nursing facilities in the Puget Sound region. This in-person role emphasizes continuity with patients and collaboration with facility teams.

You will provide comprehensive medical care for adults, manage acute and chronic conditions, and coordinate with families and interdisciplinary teams. A physician is available for collaboration and support.

Qualifications

• Licensure to practice as ARNP or PA-C in Washington state.
• Board certification in family practice or applicable field.
• Prescriptive authority as applicable.
• DEA registration or eligibility if required.

Responsibilities

• Provide primary and post-acute medical care to adults in SNF/LTC settings.
• Manage acute and chronic conditions and admissions.
• Conduct follow-up visits, regulatory visits, and discharge visits as appropriate.
• Review labs, imaging, medications, and documentation; develop/update treatment plans.
• Communicate with nursing staff, families, specialists, pharmacies, and care team.
• Coordinate care with facility leadership and other treating providers.
• Complete documentation within 48 hours, aim for same-day when possible.
• Participate in shared phone-only on-call rotation.
• Perform procedures when clinically appropriate and within scope.
