If you miss spending time and having continuity with your patients, Rejuvenation Medical may be a good fit for you.
We are a small, independent, growing medical group serving skilled nursing and long-term care facilities throughout the Puget Sound region. Rejuvenation Medical was built around a simple idea: patients in these settings deserve thoughtful, relationship-based medical care from providers who have enough time to actually know them.
Our providers get to know their patients’ goals, baseline, families, nursing teams, and facility staff. We value clinical judgment, autonomy, consistency, and follow-through rather than rushing through a list of patients.
We are growing because facilities and providers continue to seek us out, and we are looking for an ARNP or PA-C who wants to be part of that growth.
You will provide primary and post-acute medical care to adults in skilled nursing and long-term care facilities.
Most providers spend the day at one assigned facility, allowing you to develop continuity with patients and strong working relationships with the nursing and facility teams. Physicians rotate through our facilities and are available for collaboration and support.
The role includes management of acute and chronic medical conditions, admissions, follow-up visits, required and delegated regulatory visits, discharge care, medication management, and coordination with families and the interdisciplinary team.
