Qualifications

Responsibilities

• Diagnose and manage acute and chronic conditions.
• Document encounters and care plans in medical records with ICD-10 coding.
• Collaborate with nursing staff and facility teams for coordinated care.
• Educate patients and families on conditions and preventive care.
• Complete Medicare recertification visits and documentation.
