Qualifications

• Master of Social Work required.
• Minimum two years of clinical social work experience.
• One year of case management experience in acute or post-acute settings.
• Active CPR certification and renewal per policy.
• Case Management Certification (ACMA or CCM) within two years.

Responsabilités
