Job summary

Roseman University of Health Sciences is seeking a Population Health Manager to deliver social work services, care coordination, and population health support for the GENESIS program within the Office of Community Health Innovation. Qualifications

• MSW/MPH/MPA from an accredited institution required
• Minimum of three years of experience in community outreach, social services, healthcare support, or related field
• Bilingual English/Spanish strongly preferred

Responsibilities

• Social Work Assessment, Intervention & Case Management: Conduct psychosocial and social determinants assessments; develop and monitor care plans addressing healthcare, behavioral health, and social service needs.
• Care Coordination, Advocacy & Resource Navigation: Coordinate care across systems; facilitate referrals, advocate for individuals and families, reduce barriers to resources.
• Crisis Intervention & Family Support: Assess and respond to complex social, behavioral, and environmental concerns through crisis intervention and supportive counseling.
• Community Engagement & Population Health Initiatives: Support implementation of population health and outreach initiatives; develop partnerships to promote equitable access.
• Student Education & Interprofessional Collaboration: Mentor medical students in household-centered care and collaborate with faculty and teams.
• Documentation, Compliance & Program Evaluation: Maintain accurate case documentation and ensure compliance with standards.
