Roseman University of Health Sciences is seeking a Population Health Manager to deliver professional social work services, care coordination, and population health support for the GENESIS program in the Office of Community Health Innovation. Qualifications

• MSW/MPH/MPA from an accredited institution is required.
• Minimum of three (3) years of experience in community outreach, social services, healthcare support, program coordination, or related field.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Conduct psychosocial assessments and develop care plans addressing healthcare, behavioral health, and social service needs.
• Coordinate population health and community outreach activities within GENESIS; some visits may occur on weekends.
• Provide care coordination, referrals, advocacy, and resource navigation within interprofessional teams.
• Perform crisis intervention and family support services as needed.
• Support student education and interprofessional collaboration through mentorship and role-modeling.
