Title
Population Health Quality Specialist
The Population Health Quality Specialist position is responsible for overseeing Bronson Network and payor quality program performance. This includes identifying current performance, analyzing successes and opportunities for improvement, and developing and implementing improvement activities with practices and providers. The Population Health Quality Specialist will work collaboratively with payor representatives, member practices, practice management and Bronson committees and departments involved in policy, procedure and standard practice. The focus is to ensure identification and spread of best practices for meeting the goals of improved care, enhanced patient satisfaction and decreased cost for unnecessary care. The Population Health Quality Specialist is expected to have a thorough understanding of HEDIS quality measure, expertise in aspects of payor quality incentive programs and reporting websites, Bronson Network Population Health program, Bronson clinical documentation in Epic, BMS processes for improvement, and educational program development.
Bachelor’s degree required. Minimum three to five years’ experience in a medical practice or related healthcare field. Registered Nurse license desired/preferred.
- Ability to utilize computer software including word processing, spreadsheets, presentation software, electronic mail and other software relevant to the job.
- Demonstrated ability to relate and communicate well with colleagues and providers in practices and across the Bronson Network and the Bronson system.
- Effective organizational skills to manage the requirements of multiple practices and provider programs.
- Effective presentation skills demonstrating subject knowledge clearly
- Ability to identify key information in quality program changes recognizing and relating changes that impact Network and system.
- Effective problem-solving skills and conflict resolution.
Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time.
The job produces some physical demands. Typical of jobs that include regular walking, standing, stooping, bending, sitting, and some lifting of light weight objects.
- Demonstrates professional, appropriate, effective written, verbal and non-verbal communication with colleagues, vendors, medical staff and other departments throughout the Bronson Network and Bronson system.
- Good judgment required relative to sensitive and/or confidential organizational information on quality data and programs.
- Sensitivity and tact required in interfacing with practice leaders and staff, providers, and other departments within the system.
- Acknowledges patient’s rights on confidentiality issues, maintains patient confidentiality at all times, and follows HIPAA guidelines and regulations.
- Participates effectively as a team member being accountable, helpful and welcoming to all coworkers, providers and patients.
- Develop strong working relationships with payor representatives, colleagues, providers and practice leadership both internal and external to and the Bronson organization.
- Monitor Network and payor quality program updates providing key information to Network leadership, staff and provider members.
- Collaborate with Network team members in analysis of quality performance reporting and development of meaningful provider and practice reports to drive improvement.
- Monitoring provider performance identifying opportunities for improvement to achieve goals.
- Collaborate with practice leadership and providers in identification of best practices and implementation across Network.
- Support policy and procedure development to meet payor requirements and implementation of best practices.
- Develop educational tools for all aspects of the Bronson Network Population Health program to support practice and staff.
- Support Bronson Network team in payor program and initiative implementation.
- Attend system committee meetings as appropriate to support Network needs.
- Support supplemental data entry process of clinical care via payor websites.
