Job summary

Southwestern Health Resources is seeking a Program Manager - Population Health to lead deployment of population health and value-based care initiatives. The role is a full-time, hybrid position with days schedule and remote work allowed at manager's discretion.

You will coordinate cross-functional teams (Quality, Care Management, Analytics, Compliance) and manage project plans, KPIs, dashboards, and performance reports while ensuring regulatory alignment and program readiness.

Qualifications

• 5 years experience in post-acute care, care coordination, population health, or payor environments.
• Experience with project coordination or program management, quality or performance improvement.
• CAPM, LSS, CPHQ or other PM training preferred.

Responsibilities

• Lead the implementation and day-to-day management of population health and value-based care programs across the enterprise.
• Develop and maintain detailed project plans, timelines, milestones, and deliverables.
• Coordinate cross-functional teams to drive program alignment and progress.
• Track program performance against KPIs and quality metrics; elevate risks as needed.
• Partner with analytics to develop dashboards and routine performance reports.
• Facilitate regular program meetings and document decisions.
• Support gap closure and pilot programs; scale successful interventions.
• Ensure compliance with CMS, ACO, NCQA standards and audit readiness.
• Identify process improvements using data and stakeholder feedback.
