Qualifications

• Minimum education: Associate's/Technical/Vocational in Business, Healthcare Admin, Nursing, Public Health or related field.
• Valid driver's license, MVR, and vehicle liability insurance as per SFHS guidelines.
• Minimum 3 years in Care Coordination, Healthcare Operations, Population Health, or related fields.
• Knowledge of Microsoft 365 and PowerBI; familiarity with EHR systems.
• Strong communication, organizational, and relationship management skills.

Responsibilities

• Serve as primary population health liaison for assigned clinics and engage clinic leadership.
• Analyze clinic performance data and translate into actionable improvement strategies.
• Coordinate cross-functional support from Care Coordination, Pharmacy, Quality, and Risk Adjustment.
• Standardize clinic engagement processes, workflows, and best practices across clinics.
• Monitor progress toward population health goals and report updates to leadership.
