Job summary

Bayhealth Medical Center is seeking a Population Health Pharmacist to provide clinical pharmacy support for Medicare and Commercial providers, improving STAR measures and cost containment through collaboration with hospital teams.

The role emphasizes pharmacist-led MTM services, formulary education, and provider support to optimize outcomes for Delaware residents. Health plan collaboration and policy adherence are essential.

Qualifications

• Licensed pharmacist with pharmacy degree.
• One year hospital pharmacist experience or completion of an ASHP PGY-1 residency.
• Bayhealth credentialing process completion is required.

Responsibilities

• Support clinical pharmacy services for health plans and providers.
• Analyze reports and EMR data to identify opportunities for improved outcomes and cost containment.
• Educate providers and health plan members on formulary changes and MTM services.
• Participate in committees and audits to support Bayhealth initiatives.
• Maintain high STAR ratings and ensure compliance with regulations.
• Support health plan member care to prevent avoidable hospital admissions.
