Job summary

Our Client in Dover, DE is seeking a Population Health Clinical Pharmacy Specialist to provide clinical pharmacy support for Medicare and Commercial providers, aiming to improve STAR/quality measures and reduce costs. The role is full-time, Monday–Friday, with no holidays or weekends.

The candidate will analyze data, coordinate MTM services, and collaborate with care teams to drive value-based care outcomes. A BPS specialty certification and hospital pharmacist experience are preferred.

Qualifications

Bachelor degree in Pharmacy required.
Doctorate in Pharmacy preferred.
Board Certification by BPS in Pharmacotherapy, Ambulatory Care, Cardiology or Geriatrics; specialty certification required.

Responsibilities

Analyze reports and EMR data to identify opportunities to improve outcomes and reduce costs.
Assess plan members with predefined conditions; perform MTM and education; develop health improvement plans.
Collaborate with providers to implement clinical quality data initiatives and maintain 5-STAR membership ratings.
Lead committees, meetings, and audits; develop policies supporting Our Client initiatives.
Inform providers and members of formulary changes through educational programs.
Identify member needs to prevent hospital admissions; document interventions to assess success.
Serve as subject matter expert in pharmacy-related clinical measures for value-based care.
Perform other duties as assigned within scope of responsibilities.
