Job summary

Qualifications

• Bachelor degree in pharmacy is required.
• Licensed pharmacist with board certification in relevant areas.
• Pharmacotherapy, ambulatory care, cardiology or geriatrics are recognised BPS specialties.
• PharmD degree is preferred for this role.

Responsibilities

• Analyze reports and EMR data to identify opportunities to improve outcomes and reduce costs.
• Perform MTM services and develop personalized health plans for members.
• Collaborate with Bayhealth and health plans to meet STAR/HEDIS targets.
• Inform providers about formulary changes and educate on health plan initiatives.
• Support quality initiatives and maintain high patient care standards.
• Document interventions and outcomes for program evaluation.
