Qualifications

• Bachelor Degree Pharmacy
• Licensed Pharmacist; BPS Certified in a specialty, Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics

Responsibilities

• Support Bayhealth providers by analyzing reports and EMR data extraction to identify opportunities to improve outcomes and reduce costs.
• Assess health plan members with predefined conditions, provide MTM services, and develop health improvement plans.
• Collaborate with Bayhealth providers on quality data initiatives and maintain a 5-STAR rating for members.
• Participate in committees and audits; develop policies that meet legal standards.
• Inform providers and members about formulary changes through education programs.
• Identify health plan member needs and intervene to prevent hospital admissions.
• Act as SME in pharmacy-related clinical measures for value-based care.
• Other duties within scope as assigned.
