Job summary

ChristianaCare in Delaware is seeking a Population Health Clinical Pharmacist to deliver ambulatory care management and comprehensive medication reviews for patients in the Medical Group of ChristianaCare.

You will collaborate with providers, conduct risk-based population health initiatives, support CMS STAR and HEDIS measures, and participate in research as a preceptor for residents.

Qualifications

• PharmD required from ACPE-accredited school.
• ASHP-accredited PGY-1 Pharmacy Practice Residency required or equivalent (3 years clinical pharmacy).
• ASHP-accredited PGY-2 Ambulatory Care Residency preferred.
• Current licensure in Delaware or eligible to reciprocate; may require licensure in other states for virtual care.

Responsibilities

• Designs and implements clinical programs.
• Collaborates with patients, providers, and payer-based pharmacists to optimize outcomes in risk-based contracts focusing on CMS STARs and HEDIS measures.
• Provides comprehensive medication management and annual wellness visits.
• Uses risk stratification to identify high-risk patients.
• Reviews for contraindications, medications, interactions, and records in patient charts.
• Documents drug use and adverse effects; coordinates enhanced medication reconciliation for high-risk patients.
• Participates in research design, data analysis, and resident preceptorship.
• Contributes to pharmacy literature and attends regional/national meetings.
• Mentors pharmacy students and residents in ambulatory care rotations.
• Travels to practices to meet department needs.
